Executed plan document

For those of you who have submitted your executed plan - did you put your name on each page and number them?

I did neither.

Someplace in the PD stuff on the SOA website it specifically says not to include identifying information (such as your name) on the pages containing your program summaries and project information. Basically, anything that will get reviewed should be anonymous.

I did number all of my pages to ensure they would know if some got misplaced (ie Page 1 of 25).
