There are required course material and supplement course material. Do I need to buy both for the test?

You don't need to buy both. The course fee includes one attempt at the exam and access to the online material. This online material covers the test completely. The supplemental material is completely optional.

Definitely not. The online material includes a bunch of PDFs that you can view or print out yourself if you like hard copies. My understanding is that the supplemental material is pretty much just these PDFs neatly bound together.
