Blue Blank - Green Blank

Is there any written documentation on the relationship between page 4 of the Blue blank and page 4 of the green blank.

I believe that the Booke Seminar group has a lot of info on the various annual statements. I think they do mostly training and software for filling out the blanks but they might be able to point you toward the info you are looking for.
