Initial PD plan

Does anybody know if the advisor's signature is required on the initial plan? I see there's no signature on the modal plan. Should I prepare a statement similar to the one in the executed plan, stating that my advisor agreed to the initial plan?

Yes, you need the advisor's signature

My IP got sent back, because my advisor and I did not sign on the non-existent signature lines. :)
