Anyone know how they are calculating these numbers? Most of the other exhibits seem to be explained in the text.

Exhibit 15 is from the latest 2nd and 3rd reports from 1981 stat plan data; they're not calculated. You're given these numbers to compute the average cost per case (by claim and hazard group) in section 7.
