My wife does. Mostly for dictating texts and asking for info when we're on the road. (She's not the greatest typist on the Apple iOS keyboard, and bumpy roads just make it worse.)

3GS did get iOS6. It's more "in name only" because you don't get Siri or many of the other goodies, but my 3GS is running iOS6. My first generation iPad will not get iOS6, though, which irks me.

I don't know, but I think it's safe to assume it would be similar to the march 2011 deadline of mid-December. And yes, you need your paper approved before qualifying for FAC.
