No 'insight' for Insurance Companies?

Chapter 21 of Kongstvedt and MATE cards 19 and 22 say that one of the challenges that health insurance companies have is the lack of customer insight. What does this mean? Is it believed that health companies, more than other industries, are not aware of what the customer wants? Why would this be the case?
