Default Colorado's Health Insurance Cost Survey

Towards the end they are asking for "The cost of providing or arranging health care servvices". What exactly are they looking for there? Does this have something to do with Managed Care or is it something else? Seems like they are looking for an expense amount and not a benefit, but I'm not sure.
