I used diceware with real dice (not an internet generator), and created a main passphrase for LastPass, and then the rest of the passwords are generated by LastPass in obnoxious strings, that I don't have to remember.

My passwords for just about everything are the same. Some have a 1 on the end. Some have a !

One of the strongest passwords I have is for Gmail.
For facebook and goodreads, I use the same password, not related to the one for Gmail.
All my other passwords are written down somewhere so I don't have to remember them in between times I am actually using them.
