Default Service Table

Hello,
I have to calculate the present value of a pension plan. Therefor I have to use mortality, invalidity and withdrawal rates to construct a service table for this decrements, that a beneficiary doesn't stay until his retirement age in the company. But I never constructed a service table. Can somebody point me to a webpage where the construction of a service table is explained? Gracias!
