Another underrated breakfast option is Chick-fil-a. Not only are the hashbrown scrambles delicious, but they'll make anything on the menu so if you want a chicken sandwich at 7am you can have it.

I voted for Le Peep (I love their skillets), but I also really like Perkins and IHOP. I'm also fond of fast food breakfast - McDonald's, BK, Hardee's.

My answer from when I worked at McD's 20+ years ago is that there was not room in the grill area to accommodate both breakfast and lunch stuff, both from a storage and grill surface perspective. This was also back before everything was made on demand, so they probably would have thrown away a lot of burgers in the morning (that would probably be a more volatile demand curve). Given that they now make everything fresh and have certain all-day breakfast items though, that hurdle has clearly been overcome. These days, there's probably just not enough demand for burgers at 7 am to justify having all the necessary ingredients (including thawed hamburger patties, which can only be kept so long) available.

If we are going non-primarily breakfast chains, then the breakfast sliders at White Castle are the best. I used to occasionally get a box of frozen ones at the grocery store but I don't think Kroger carries them anymore.

As I understand it, a big part of the issue stems from cooking temperatures. I guess that goes into room in grill area / surface + demand for burgers, but if the whole place is set to a lower temperature grill to produce for breakfast demand, it would really suck to take an entire grill, heat it up to burger / other food temperatures, cook that one order, then wait for it to go back down.
