That was quite a while ago (early 90's IIRC). It was in Toronto and is seems that when the exams (Part 5?) were bundled up to be couriered to the CAS offices they wouldn't fit in one courier envelope so they were put into two envelopes that were then taped together. Somewhere in transit the envelopes came apart and one package went missing.

The CAS did let the affected people sit another exam - given the short time span between the original exam and the resit they simply reused questions from past exams. Anyone who figured that out and studied accordingly passed.

FedEx lost a handful of exam 6s last year. They were eventually found, but after the candidates sat for a makeup exam. I believe I heard the 8 people who sat for the makeup passed.

pretty unexcusable imo. nowadays all packages can get traced electronically etc, something getting lost in the mail so to speak sounds like someone didn't try hard enough for find the lost materials.

Eh, UPS tracks everything, but they still run a whole insurance shell to self-insure themselves for lost packages. There's always room for some error.
