Costs are higher for the no-contraception plan because of all the extra babies?

It's a pity we are conditioned to expect flat rates for families. What's so wrong with charging a premium per child? edit: There would also be the costs associated with pregnancy itself, but charging per head would help somewhat, while avoiding th political minefield of what constitutes "equal" coverage when the concept itself isn't even relevant in the first place.
