Other things I would wonder about:

How low, BMI-wise, does the "skinny" group go? Are the medially-underweight being lumped in with those in the "normal" range?

Possibility of a correlation issue - underlying factor is "being really, really, extra sick", which might separately cause both low weight and greater post-surgery death rates.

The people that do studies should hire an actuary to think of stuff like this.

My guess is that the skinny people are dying more often from the strain of having sex with the nurses. The nurses don't want to have sex with the fat ones.
