Default Disease Management Profiling

I have about 50 disease management programs that I'd like to evaluate based on quantitative and qualitative measures. I've brainstormed this for a while and came up with the following 8 measurements:

Resources needed
Savings (projected)
Quality
Technology used
Employer & Customer Demand
Measurability
Developmental Status
Intensity

Has anyone ever cataloged DM program, if so, did you use any other measurements? Any feedback is greatly appreciated.
